Medicaid Services Coordination

The service coordinator partners with you and others you choose to develop, implement, and maintain your Individual Service Plan (ISP). Self-advocacy, empowerment, choice, self-direction are encouraged throughout the person-centered planning process. This partnership is built on mutual respect, understanding and accountability.


 The service coordinator will:

  • Assist you to obtain eligibility for services
  • Assist  you to apply for & keep your benefits & entitlements, (Medicaid, Social Security, etc)
  • Assist you to explore interests & experiences to determine goals
  • Assist you in understanding what services are available
  • Connect you with supports & services to meet the goals of your Individual Service Plan
  • Assist you to apply for and obtain approval for services that you want or need
  • Assist you to secure funding for services, as needed
  • Link you to resources 
  • Promote informed choices
  • Honor your preferences
  • Advocate for you
  • Monitor your health and safety
  • Listen to you
  • Communicate with you
  • Assist you to solve problems and learn ways to solve problems on your own. .
  • Monitor your satisfaction with services & help you to make changes, as needed
