Service Coordination


This program provides assessment, linkage and referral to support each individual's determined service plan.

Heritage Centers Service Coordination can provide: coordination of health services; assistance with obtaining appropriate living arrangements; coordination of educational, vocational and transitional placements; assistance in obtaining/maintaining benefits and financial entitlements such as Medicaid and SSI; coordination with early intervention; linkage and referrals to respite, residential habilitation and recreation programs.

Service Coordination also assists in the development of individual service plans (ISP) and coordinates the treatment team for annual and semi annual reviews.
